"This article is the long version of a conference given to Columbia University of New York on September 21st, 2009, on the reform of the health insurance in the United States where the debate concerning the creation of a public and compulsory insurance rages.

"The French systems and American are compared around the major axes of the reform which the president Obama defends with pugnacity, in particular the creation of National Insurance Exchange where would reign finally a free-market economy over the market of the health insurance, convenient to a decline of the premiums.
